Amendment of the Rule adding the requirement of the use of an approved form and establishing a $30.00 filing fee was made pursuant to the General Order of 3 October 1996.
Effective date: 7 October 1996
General Rule 3.12 was further amended to increase the fee for pro hac vice admissions from $30.00 to $100.00 pursuant to General Order of 2 July 1997. This increase made the admission fee for pro hac vice admissions the same as that for admission to the general bar.
